Die Entwicklung des eine HIV-Infektion verhindernden Medikaments Lenacapavir ist für das renommierte Fachmagazin «Science» der wichtigste Forschungsdurchbruch des Jahres. Damit werde der nächste, aber keineswegs letzte Schritt im Kampf gegen Aids gewürdigt, heißt es zur Begründung des «Breakthrough of the Year».

Weltweit leben nach Daten des UN-Programms UNAIDS rund 40 Millionen Menschen mit HIV, der Großteil in Afrika südlich der Sahara. Eine halbjährliche Spritze mit dem Medikament Lenacapavir schützt effektiv vor einer Infektion mit dem Virus, wie Studiendaten zeigten. Bisher verwendete Mittel zur HIV-Präexpositionsprophylaxe (PrEP) wie Truvada müssen täglich als Tablette genommen werden.

Die Entwicklung des Medikaments stelle einen ähnlichen Fortschritt in der HIV-Prävention dar wie frühere Durchbrüche mit antiretroviralen Medikamenten, erläutert «Science»-Chefredakteur Holden Thorp. «Die sechsmonatige Wirkungsdauer von Lenacapavir macht einen großen Unterschied und bietet eine neue und bessere Möglichkeit, die Prophylaxe mehr Menschen auf der ganzen Welt zugänglich zu machen.»

Wertvolle Verbesserung

Sich zweimal jährlich spritzen zu lassen, sei wesentlich komfortabler als täglich an die Einnahme einer Tablette denken zu müssen, betonen Experten. Hinzu komme, dass es gerade in einigen stark von HIV betroffenen Ländern bei der täglichen Einnahme von Tabletten das Risiko gebe, im Umfeld als vermeintlich HIV-positiv abgestempelt zu werden. Eine nur zweimal jährlich verabreichte Spritze sei da sehr hilfreich.

Der Hersteller Gilead will die Zulassung als HIV-Schutz in zahlreichen Ländern beantragen. Gezielt werde an einer Versorgung auch in ärmeren Ländern gearbeitet. Das Mittel soll prophylaktisch Menschen mit hohem HIV-Infektionsrisiko angeboten werden. 

In der EU ist Lenacapavir zur virushemmenden Behandlung bestimmter Patienten zugelassen, die schon infiziert sind. In Deutschland wurde das Medikament dafür bisher von Gilead nicht auf den Markt gebracht. Ob es als vorbeugendes Mittel hierzulande erhältlich sein wird, ist unklar.

Dritte Ehrung für Forschung zum Thema

Dass das HI-Virus die Immunschwächekrankheit Aids verursacht, ist seit Anfang der 1980er Jahre bekannt. In der Folge wurden antiretrovirale Präparate entwickelt, die den HIV-Spiegel so weit senken, dass die Übertragung gestoppt wird.

Es ist das dritte Mal, dass «Science» eine Aids-Intervention als Durchbruch auszeichnet. Im Jahr 1996 markierte demnach die Entwicklung von Proteasehemmern als wirksame Waffe gegen HIV einen Wendepunkt. «Bei den meisten Patienten konnte diese Klasse antiretroviraler Medikamente in Kombination mit anderen antiviralen Wirkstoffen den HIV-Spiegel im Blut drastisch senken», heißt es von «Science».

Im Jahr 2011 habe dann die bahnbrechende klinische Studie «HPTN 052» gezeigt, dass die Behandlung mit einem Cocktail dieser Wirkstoffe auch das Risiko einer sexuellen Übertragung des Virus auf andere Menschen erheblich verringert.
